Appointments Appointments may be scheduled by calling 513-889-2554 Monday to Friday 9:00 AM to 4:30 PM You need to bring any records, X-rays, MRI and tests you have to help diagnose and treat you. If you need to cancel or reschedule your appointment, please do so in time so we can use this time to see some other patient. Thank you.
Emergencies Please go to Emergency room or call 911 for life threatining emergencies or call us in office hours for directions.

Fees and Payments We accept most Insurances. Please contact our office.
PRESCRIPTIONS & REFILLS If you need medication refill, please call 513-889-2561 or call your pharmacy and they will fax over your request to our office. Refills must be made during office hours. Please allow 48 hours. If you are taking controlled substance ( Except Vicodin , Loracet , Norco and Darvocet ), the treating physician will have to write the prescription and you will have to pick it up. If you have any side effects to your mediccation, please contact the physician or office. In case of life threatening emergency, please call 911.
